Clearing the Cloudiness That Comes Back After Cataract Surgery

Cataract surgery is one of the most successful operations in all of medicine — but for some patients, the vision that was restored so beautifully after surgery gradually becomes cloudy again months or even years later. This is not the cataract coming back. Why Does Vision Become Cloudy Again After Cataract Surgery?

During cataract surgery, the cloudy natural lens is removed but the thin, transparent bag that held the lens — called the posterior capsule — is left in place to support the new artificial lens. In around 20 to 40 percent of patients, cells from the edge of the original lens gradually migrate across the back of this capsule and make it cloudy. The result is a gradual return of blurred, hazy vision that can feel exactly like having a cataract again — even though the original cataract was successfully removed.

What Happens During YAG Capsulotomy?

Step 1 — Dilating the Pupils

Eye drops are used to dilate your pupils fully so the laser has a clear view of the posterior capsule behind the artificial lens.

Step 2 — Anaesthetic Drops

Numbing drops are applied. A contact lens may be placed on the eye to help focus the laser precisely.

Step 3 — The Laser

The YAG laser creates a small, precise opening in the centre of the cloudy posterior capsule. The laser pulses take only a few seconds in total and the procedure is completely painless. You may see brief flashes of light as each pulse is delivered.

Step 4 — Done

The entire procedure takes five to ten minutes. Most patients notice an improvement in their vision within a few hours. There are no stitches, no incisions, and no recovery period.

After the Procedure

Your vision may be slightly blurred for a few hours as the dilating drops wear off. Some patients notice small floaters after the procedure — these are fragments of the capsule that have been cleared away and are completely harmless. They typically settle out of the central vision within a few weeks. A brief follow-up is arranged to confirm your vision has cleared fully and your eye pressure is normal.
